- Clark County resident
- Children age birth to three years
- Diagnosed developmental disability or a significant delay in one or more areas of development
- Services and supports are based on the child's individual needs
- No waiting list allowed

- Referrals made by parents, health care professionals, or they may come as a result of a countywide development screening.
- B-3 coordinator will contact the family to see what the child's needs are.
- After a child is determined to be eligible, evaluations are done, and an individual plan is written.
|
- Fees will vary depending on the types of services that are needed.
- Eligibility is not affected by the family's income; however, parents may need to contribute to the cost of services.
|
Family Support Program:
Supports and funding for services and goods needed for families with a child with severe disabilities living at home. Funding is limited to $3000 per year.

- Clark County resident - families are eligible if they have a child with a severe disability (per state definition), under the age of 21, and living at home.
- Eligibility is not affected by the family's income.
- Because each county has limited funding for this program, eligibility does not guarantee a family will receive services.
- Agencies may have waiting lists for services and assessments. Priority may be given to families in a crisis situation.

- Families apply for services by contacting their Family Support Coordinator.
- Application process includes an eligibility determination and a needs assessment.
- A home visit is scheduled, and a service plan is generated.

No income limit; however, parents may need to contribute to the cost of services.

Long Term Support Programs:
Support, services, and funds to individuals to help people live in their own homes and communities rather than being institutionalized in order to receive the long-term care they need.

- Clark County residents of all ages; requires a specific level of function and care as defined by the program.
- Income and asset guidelines are similar to financial eligibility for Medicaid.

- Referrals can come from school districts, family member/guardians, individuals themselves, and other sources, and should be made at least three semesters prior to graduation (if the individual is still in school).
- An intake assessment will be done in order to determine eligibility.
- School records and other psychological evaluations may be gathered at this time.
- Because of limited county funds, there may be a waiting list for services.

- Fees vary depending on the types of services that are necessary.
- State and federal funding programs are also utilized by individuals who are found to be eligible for those particular programs
